New Choices Waiver
This program is designed to financially assist individuals residing long term in a nursing facility who wish to live in a community setting. Community settings include assisted living facilities, independent living facilities as well as living at home. NCW covers services that go beyond what regular assisted living provides, giving individuals more choice and control over their care while offering extra support so they can live independently and comfortably. Together, our team of nurses and social workers play a crucial role by assisting clients in locating, accessing, coordinating, and monitoring those services available to them.
Clients on the Utah Medicaid New Choices Waiver are eligible for all the services listed below, but each client is required to have a Case Management Agency. At Utah Case Management, we are committed to finding the perfect plan of care for you as your case management provider.

How To Enroll With UCM
STEP 1
If you are in a nursing home or assisted living facility, connect with your resident advocate or admin. If you're at home simply call or email us to fill out an application.
STEP 2
Select “Utah Case Management” as your case management agency in the application.
STEP 3
A Utah Case Management social worker and nurse will contact you for an initial assessment and begin the coordination of care process.
STEP 4
Your case managers will help you personalize a care plan that best suits you!
