Department of Services of People with Disabilities
At Utah Case Management, our support coordinators collaborate with their client to help create a comprehensive care plan best suited to their needs. We take a holistic approach in joining forces with everyone that is a part of the care plan to incorporate client’s ambitions, dreams, wants and needs. Whether in a day program or a group home, if our client is not happy with their current services, our support coordinators will advocate on their behalf and find other acceptable services.
Using these waiver resources listed below, and a variety of other non-waiver resources, our support coordinators can create a personalized plan and set specific goals for their client to have the best quality of life.
