At Utah Case Management, our support coordinators collaborate with their client to help create a comprehensive care plan best suited to their needs. We take a holistic approach in joining forces with everyone that is a part of the care plan to incorporate client’s ambitions, dreams, wants and needs. Whether in a day program or a group home, if our client is not happy with their current services, our support coordinators will advocate on their behalf and find other acceptable services.
