Housing Related Services and Support

The HRSS program is available for those members on Targeted Adult Medicaid experiencing homelessness, food insecurity, interpersonal violence, substance abuse, or mental health issues. Our Case Managers work to find their clients a safe home and a stable job to match their skills and interests in a role they enjoy. With a holistic approach, the client’s ambitions, dreams, wants and needs will always be at the forefront of their personal development plan. Previously incarcerated individuals can also be eligible for HRSS support and will find the same attentive assistance from our Case Managers. We are prepared to help you create goals for your future success!

We do this by:

Weekly Case Manager Visits

Personal Development Planning

Weekly Educational Classes

As well as collaboration of care and or setting up services with the following health providers:

Mental health

Substance abuse 

Community and peer support

Crisis intervention

Transportation

Medical & Dental

Community Transition Services

These are funds available to clients experiencing financial barriers preventing them from safe and comfortable housing. Funds can be used for the following:

Security deposit financial assistance

Essential household furnishing and supplies

Covers set-up fees or deposits for utilities

Moving expenses

Work licensures

DMV licensures and more

Our Case Management Services is a range of services designed to help people in maintaining their housing and living independently. These services aim to help tenants sustain stable housing, prevent eviction, and improve their overall quality of life.

Eligibility

Individuals must be enrolled in Targeted Adult Medicaid (TAM), meet at least one of the Needs Based Criteria and at least one of the Risk Factors listed below:

Needs Based Criteria

  • Requires improvement stabilization or prevention of functioning resulting from substance use disorder, serious mental illness, developmental disability, cognitive impairment, behavioral impairment resulting from dementia, brain injury or other medically based behavior condition/disorder: OR

  • Requires assistance with one or more activities of daily living (ADL's), one of which may be body care, verbal queuing or hands-on assistance.

Risk Factors

  • Living or residing in a place not meant for human habilitation, a safe haven or in an emergency shelter continuously for at least 12 months or on at least four separate occasions in the last three years.

  • Currently living in supportive housing but has previously met defined risk factor above.

  • Has successfully completed a substance use disorder treatment program while incarcerated in jail or prison, including tribal jail.

  • Was admitted to (and discharged from) the Utah State Hospital due to an alleged criminal offense.

  • Has been involved in a drug court or mental health court, including tribal courts.

  • Receives general assistance from the Utah Department of Workforce Services; OR

  • Was civilly committed to (and discharged from) the Utah State Hospital.
