Medicaid is a state/federal program that pays for medical services for low-income pregnant women, children, individuals who are elderly or have a disability, parents and women with breast or cervical cancer. To qualify, these individuals must meet income and other eligibility requirements. Learn more on how to qualify for Medicaid here.

First, obtain a New Choices Waiver application by calling Utah Medicaid
(801-538-6155), or through the assistance of nursing home or assisted living staff. When prompted for a case management agency, choose ‘Utah Case Management’ in the New Choices Waiver application process. A Utah Case Management social worker and nurse case management team will contact you for an initial assessment and determine your eligibility. Case management is a method of providing services whereby a professional social worker and or nurse assess the needs of the client and the client’s family, when appropriate, and arranges, coordinates, monitors., evaluates, and advocates for a package of multiple services to meet the specific client’s needs.
