We specialize in collaborating with multiple community and health resources to find the perfect ‘Plan of Care’ for each individual need which allows our clients to be successful in their community whether in an Independent or Assisted Living environment or in the comfort of their home. We take pride in having fewer clients per case manager which allows more one on one attention and regular follow up with all the different service providers for our patients. Medicaid is a state/federal program that pays for medical services for low-income pregnant women, children, individuals who are elderly or have a disability, parents and women with breast or cervical cancer. To qualify, these individuals must meet income and other eligibility requirements. Learn more on how to qualify for Medicaid here.

First, obtain a New Choices Waiver application by calling Utah Medicaid
(801-538-6155), or through the assistance of nursing home or assisted living staff. When prompted for a case management agency, choose ‘Utah Case Management’ in the New Choices Waiver application process. A Utah Case Management social worker and nurse case management team will contact you for an initial assessment and determine your eligibility. Case management is a method of providing services whereby a professional social worker and or nurse assess the needs of the client and the client’s family, when appropriate, and arranges, coordinates, monitors., evaluates, and advocates for a package of multiple services to meet the specific client’s needs.
