We'll Make it Right.

Meet The Team

At Utah Case Management we specialize in collaborating with multiple community and health resources to find the perfect ‘Plan of Care’ for each individual need which allows our clients to be successful in their community whether in an Independent or Assisted Living environment, or in the comfort of their home.

We are a family owned and operated company with over 20 years of case management and nursing experience. We are licensed bonded and insured. We take pride in having fewer clients per ‘Case Manager’ which allows more one on one attention and regular follow up with all the different service providers for our patients.
